agree Marcelo Silveyra : I just want to mention the possibility that the expression depends on the country/region. I just had surgery in Los Angeles a couple of weeks ago, and the instructions said "do not eat or drink anything"...nothing about "fasting" in this particular case

I lived in an English speaking country for seven years (I am also married to a native English speaker)and "must/have to fast" is to me the natural expresion
