Or you bring medical card...but these days the systems work in wondrous ways in different countries:) so i'm not entirely sure

medical history
I see it as a narrowed-down, abstrated form of medical records, often given to the doctor to fill out for an potential employer, or given by potential employer. It contains major major life-long medical information, etc.

actually, this one is not the correct one, as it pertains to the doctor himself, but I still stand behind my entry as to 'medical history'...only because it is a briefing about a person's general medical history
