Я бы перевела весь пассаж
стресс или сильное психологическое волнение
как
physical or psychological stress
или как
physical or emotional stress.

Во всяком случае, в обратную сторону emotional/psychological stress очень часто переводится как
сильные [душевные] переживания/волнения/перенапряжение и так далее. А под просто stress очень часто имеется в виду физическое перенапряжение, чрезмерная физическая нагрузка.

не могу утверждать со всей ответственностью, однако из данного определения не следует, что anxiety - не волнение. волнение так же является обобщенным состоянием и также не имеет направленности на конкретный объект и событие
agree Alexander Demyanov : I was just going to suggest "anxiety" myself but a quick research showed that it carries negative connotation whereas "волнение" doesn't necessarily imply a directon ( or -)++On 2nd thought, in the context, "волнение" does.

severe (psychological) agitation

I think this fits best with 'stress' in this context. Possible alternatives are 'severe anxiety' or even 'severe mental anguish.'

The word 'unrest' is most often used in the context of civil unrest, ie. riots in the streets. It could be used to refer to a person's state of agitation or restlessness, but I don't think it would ever be used this way in a cardiology report.
