While these local reviews and ratings can be the lifeblood of small businesses, they can also have an enormous impact on regional, national, and even global concerns, as their larger reputations are pegged to customer opinion at the local level.

to flag = to slow down

Judging by the way you have parsed this, I think you may not have realized that 'flag' here is a verb, not a noun.

So nationwide management can notice a slow down / downturn / fall in sales from any one shop due to bad publicity locally; in other words, it doesn't only affect the local store itself, but gets noticed by top managamen at national level.
