German term or phrase: Zersplitterung der Zuständigkeiten
In this context:

This part of the article is about why Germany is lagging behind other countries when it comes to organ donation.

Paragraph:

I have found 'fragmentation of responsibilities', but am not sure this fits here.

Children's trusts have been created to address the fragmentation of responsibilities for children's services. They build upon, bring together and formalise the joint work that is already taking place in many local areas."

"competencies" is definitely not among my favourite terms but nevertheless ok here IMO and I prefer "division" to"fragmentation" but let's see what others have to say ...

"competencies" is really horrible "technospeak" but used a lot

your own "responsibilities" (also suggested by Steffen) would work too

fractured responsibilities

Explanation:
See link below. Applies to the health sector as well:

"According to the Federal Network Agency, the reasons behind these delays relate to public opposition at the local level (e.g. with respect to overhead power lines), fractured responsibilities for site approval and in some cases changing procedures."

Plenty of hits for this term. Here's another one:

"At the state level, similarly fractured responsibilities are common. In the state of Virginia, for example, the Department of Environmental Quality permits wastewater while the Department of Conservation and Recreation handles stormwater permits."

neutral  Steffen Walter: "Fragmented" rather than "fractured". Your reference is a translation (note the ref. to the "Bundesnetzagentur"). / Perhaps it's a US vs. UK thing - "fractured" had struck me as unusual in this context.

-> I disagree. This term is really much too common to be hinging on a single source. See note//I woudn't rule that out although it's used outside the US as well.

neutral  Cilian O'Tuama: 180 is plenty?// Aha, so it's yourself you hear using these strange expressions so often

-> I have little need for counting beans with expressions I use every day//If this is strange to you, that's hardly my problem. I am not responsible for other people's limitations.
