I think 'Spitzen-' refers to the top of the organisational hierarchy, hence 'national' fits best IMHO.

in analogy with the first reference, that would be my choice for the difficult to handle "Spitzenverbände der Krankenkassen". However, this term often remains untranslated and probably should at least be provided between brackets.
