There are three booklets: ITP and You, IEP and You and one about parents' rights.
And also according to the other translator's website, I am highly sure that YOU cannot be separately translated as a booklet name, pls see below for detailed information:

The information in contained in the {ITP and You} has been developed to help students with disabilities (beginning at age 14 or younger if appropriate) take part in the Individualized Education Program/Individual Transition Plan (IEP/ITP) team meeting.
