Вообще, мне кажется в Вашем конексте, оптимальный вариант - предложенный Мишей.

"составлен и подписан" обычно пишем, когда говорим, что документ составлен на двух языках, например "составлен и подписан на английском и русском языках".

Asker: Уважаемая Надя! Далее по тексту следует удостоверительная надпись и печеть нотариуса. Т.е. если это заявление кто-то и оформил, то это нотариус, а не, а не Member, который составил его и подписал. Хотя еще сегодня утром в другом документе я именно так и перевел: "оформлено и подписано". Привет Мише., Салют

disagree  Yuri Zhukov: Почему "составлен и подписан"? Здесь что-то не то. Сначала документ составляют, а уже потом подписывают, а в оригинале "подписан и составлен". Как это объяснить.
