Disagree with "transexualna orientacia". Transexuality is not kind of sexual orientation. A man or a woman becomes transexual after the sex change (the operation resulting in change of gender).

You could probably also use "zmena pohlavia" (sex-change) depending . "Transexualita" is used and understood in Slovak.
If not sure try "transexualita (zmena pohlavia)", thus using both terms.

After further research, I think that the ideal translation would be: "...pohlavia, sexualnej orientacie, pohlavnej identity (napr. transexuality, transvetizmu), rasy....." Back translation:"sex, sexual orientation, gender identity (e.g.transexuality,transvetism). race..."
You would need to talk to the client as transvetism is not included in the original text, but it would make sense as most pre-op transexuals dress like the opossite gender

"transsexualita" also seems quite valid, though it hasn't entered the official lexicon. In the genitive form required by the sentence, "transsexualizmu", or "transsexuality" to use Rad's reply. -alita actually sounds more natural by analogy to "sexualita".
I also (weakly) agree with Rad's position on "transsexualna orientacia" in a very strict sense, though since it sounds like you are primarily interested in the correct English rendering of a Slovak term in the context of non-discrimination, it's largely an irrelevant point.
IMHO, this question would have been better posed as SK-EN rather than EN-SK, and with the Slovak source context alongside.
