is me changing the plastic bags in his abdomen
every two hours so his shit wouldn't leak.
That was like a double twist on the high degree
of difficulty on that insult dive.

double noeud
C'était comme faire un double nœud de difficulté maximale pour ce qui s'apparentait à une plongée dégradante.

Discussion
Kwoning this would then help to understand this 'double twist' — I think this must be in the sense of 'a twist in something' (a plot or a tale, for example — in the sense to some extent of a 'tournant'); in which case a 'double twist' might mean some kind of double irony, for example... but we really can't tell without knowing what the situation is, and what has gone before; it might for example help to know who the he is in 'his abdomen'?
I can imagine a scenario where someone is criticizing someone else for not having done something for their late parent, and the first person retorts that they were at least there to perform this unwelcome task.
C'était comme doublement surajouter de la difficulté à cette déjà haute plongée/immersion dans la difficulté
