This is a hospital report and I am not sure if is "H." used to mean Hospital, but this is my try:

H. Día de Oncología is not the name of the hospital, it is a unit or center that provides care during the day, and which is part of the hospital, thus "H. Día de Oncología de [HOSPITAL]", which means you have to enter the name of the hospital [whatever hospital it is] on the form.
You may well be right, but in this case it does not sound like a proper name.

Em Marco de 1998 foi concluída a segunda grande obra de ampliação do Hospital, com a construção de um edifício ligado aos existentes. Trata-se de um edifício com seis pisos e uma área de construção de 4225 m2 que instalou os serviços de internamento (medicina interna, cirurgia geral, pediatria e ortopedia), a medicina física e reabilitação e a esterilização.
