Italian to English Medical Medical: Cardiology Summary of exam
It appears by itself at the end of an exam summary. It was in all caps so I left it as such (I wasn't sure about the abbreviation E.O.) Thanks for your help!

The entire term should be translated to make sense.
As we are speaking about a cardiologic visit, obviously it is about heart sounds (at auscultation).

Means they can still be heard but not as intense as in previous occasions
