　鬱病患者が急増する中、医療機関などで復職支援（リワーク）のプログラムが広がっている。厚生労働省の調査では、鬱病などを含む気分障害の患者は推計で、平成８年は４３・３万人だったのが２０年には１０４・１万人と、１２年間で２・４倍。鬱病は、いったん復職しても再休職する例も多く、再発予防が大きな課題となっている。（油原聡子）

【表で見る】　復職支援プログラムの流れ

　◆会社を疑似体験

　メディカルケア虎ノ門（東京都港区、五十嵐良雄院長）は１７年、復職支援専門のリハビリテーション施設を国内で初めて開設した。昨年１２月末までで、５９７人が復職支援プログラムを終了し、５８５人が復職。復職者の再休職率は半年で６割といわれているが、終了者の就労継続率（推計値）は１年後で８１・５％、２年後は７０・１％だという。

　リワークプログラム（デイケア）は週２日（半日と１日）から始まる。自分の病気を理解し、集団生活に慣れるのが目的だ。会社生活と同じように午前８時半に始まり、週５日安定して通えるようになると、企業と復職について相談。早い人で６カ月程度だという。

　保健師など専門スタッフのもと、仕事で想定されるチームを組んで作業に取り組むなど、復職後に想定される事態に対応できる力を身に付ける。福島南事務長は「同じ病気の仲間がいることで、復職へのモチベーションも保てる」と話す。

　これまで、鬱病の治療は薬物と休養が中心だった。だが、五十嵐院長は「薬物と休養だけでは不十分」と指摘する。

　診察程度の短い時間なら、患者が調子がいいように振る舞うことが可能だからだ。だが、同院では１日６時間のプログラムでの様子を観察するため、五十嵐院長は「プログラムは治療の一環。長時間接すると、回復状態がより正確に分かり、治療にも生かせる」と説明する。

　医療機関での復職支援は広がっており、うつ病リワーク研究会（港区）によると、現在、１２０を超える医療機関で復職支援プログラムが行われている。

　◆病気を隠して

　仕事を辞めた鬱病患者の支援も始まっている。再就職支援に特化した施設「ハビトゥス市ケ谷」（新宿区）はグループワークを行い、患者同士が悩みや目標を共有し、再就職を目指す。現在、問い合わせが相次いでおり、見学や体験を断っている状態だ。

　昨年１０月、鬱病でコンサルティング会社を退職した女性（３９）は「人とかかわることで、自信を取り戻すことができた」。現在、就職活動中だ。

　求職中の鬱病の患者は、病気を隠して就職活動を行うケースがほとんどのため、就職しても周囲の配慮が得られず、再発しやすいという問題がある。

　施設を経営するリヴァの伊藤崇社長（３４）は「鬱病の人はまじめで能力が高い人が多く、復職や就職できないのはもったいない。今後は、病気を開示しても就職できる企業が広がるように活動していきたい」と話している。
