Given the context (i.e. breast cancer/silent killer), I would go for standard terminology such as "noticeable" or "obvious".
Of course, you could rephrase your sentence and have "silent" (the most common term used):

striking symptoms

"Noisy" symptoms is obviously not an option. If you want to be poetic, you could say "clamorous", but I think "striking" is better.
