como en ambos casos separa con "or" dos partes, yo entiendo que la primera parte -delante de ambos "or"- se refiere a las muestras de aspecto más claro y la segunda -detrás de "or" en ambos casos, a las muestras turbias

sobre todo (y además) porque una muestra turbia también tiene un color anormal.

  -> al ver la frase al completo, entiendo que la primera parte "off-color" se refiere a las muestras hemolizadas y lipémicas (más claras de lo normal) y la segunda (más turbia) a la bilirrubina.
