Mobility problem within the home

It means you have difficulty in getting from one place to another round the house, e.g. arthritis in the joints.

explanation below

The person is asked whether they experienced any difficulties getting/moving around the house, difficulties which they might not have had before the disease. Having the disease seems to imply that it could be more awkward for the patient to cope in the house (probably with regard to walking stairs, moving furniture, washing floors, etc)
