I think "surgical record" is best for a document detailing everything that was done (seems to be the case there), including anesthesia, biopsy reports etc.

surgical operation

I think atto stands for operation in this case, surgical operation.
If you google, you'll find that the expression "atto operatorio" is a bureaucratic expression for that.

I think it simply means "operazione", the surgical procedure for remedying an injury,etc..
i'd choose operation instead of surgery since
OPERATION is the generic term
SURGERY is a "surgical operation or procedure, especially one involving the removal or replacement of a diseased organ or tissue"

I think that you nearly had it with your suggestion. But since this clearly deals with a single surgery act, I would ad the genitive
