It's a Form (Modulo) issued by ACI/PRA called . Dichiarazione di vendita. The reason for the sale is: CESSAZIONE CIRCOLAZIONE PER ESPORTAZIONE.

The portions of the sentence I do not understand is: "verbalmente venduto". I'm rather confident it does NOT mean s"old verbally", i.e. without a written document.
