Impossible to answer without knowing what transition process you refer to. Which country? Transition from what to what? What part of the sentence are you having trouble understanding (particular word, term, etc.)?

Helping an Exceptional Student Prepare for the Future
As part of the Individual Education Plan, a plan for the student’s transition from secondary school to a postsecondary setting must be developed with input from the student, parent(s)/guardian(s), the principal, school staff, community agencies, and postsecondary institutions, as appropriate. The plan should reflect the student’s needs and goals for his or her future as per the IEP Standards. The transition plan must include the following elements:

specific goals for the student's transition to postsecondary activities. The goals must be realistic and must reflect the strenghts, needs and interests of the students;
