In the UK at least, you can refer to a "pension scheme" which covers all possibilities from those run by individual companies to the state scheme.
This is realy your "regime de retraite"
The "caisse" is literally "fund" as DrPC says, but in English it would sound very strange to submit a request to a "fund", so I would probably say "provider" or possibly "administrator".
But again, more context would help enormously!

OK, following your latest additional note:
You may or may not know, but pension arrangements in France are organised according to your professional/employment status. So, basically, an employee pays into one fund, the self-employed into other funds (depending on what they do), various professions have their own funds, etc. This accounts for the reference to the \"caisse du regime de votre derniere activite\". But, in more generic terms, all this means is \"the last (pension) fund that you belonged to\".
So, given the full context (finally) plus Patricia\'s comment plus the knowledge that you need it for the US, I think I\'d put something like:
\"Your request should be submitted to (the provider/administrator of) the last pension plan that you belonged to. However, you may submit it to one of the other aforementioned (pension) plans of which you have (previously) been a member.\"
Suspect the last sentence probably needs a bit of work (!), but that\'s the meaning.

: and in U.S./Canada, "scheme" would be replaced by plan -- to go with Charlie's good idea for a generic term -- also like administrator in the right context
