I know there are lots of entries for "scarico" in the glossary already, but I'm not sure about what it means here. It's an item on a price list for a prepaid card - see http://www.icbpi.it/Trasparenza/Doc/PERFECTO_DS.pdf for a similar context to mine.
My tentative translation is "Card discharge", but I'm checking because I'm not sure just what this means - in Italian or in English.

transfer funds from prepaid card to your bank account
If they are referring to transfer from card to a bank account, then it's that, a "transfer". But if they mean the diminishing value of the card as you spend money on it, then I would say "debit charges".
scarico: moving money from the card. Decrease value on the card.
carica: moving money to the card. Increase value on the card.

I looked at your context doc and since this deals with credit cards and not paper, I would say something like card disactivation (fee).
Just one idea :)

: When you "scarichi" you transfer part or all of the balance to your bank account.

Comment: "I think estinzione, which came up later in this translation is actually the Italian for disactivating a card or closing an account. Thanks anyway."
