Therefore, yes, I think you are right about erhaltene and David is right that the -e should not be dropped. The easiest way to deal with this is to use "assigned," IMO:
"Record the number assigned to the patient by the hospital admitting office. If the hospital has a unit numbering system, all patient records will carry this identifying number. If the hospital has a serial numbering system, a new number is assigned on each admission to the hospital."

Hi Sonja, I'm not sure that "erhalten" fits here, either. I could suggest "erhobene", which here looks as if it might well mean "recorded", but this is only surmise. To drop the "e" off erhalten seems to disagree with the "Nummer".
To what does the "Aufenthalt" refer? Holiday, or a stay in hospital? Do give us more context, e.g. the origin of the letter and its addressee. Is it a Krankenkasse, travel agency or what?
