This has come up a few times in a translation I am doing about liposuction procedures. I understand what geste chirurgical means but I haven't been able to find a good reference for exactly what would be meant by "geste fort" in the context of cosmetic (or other) surgery. It's part of a table listing pros and cons of liposuction techniques, so there's not a ton of context, but it's listed as a con under the "traumatisme" criterion.

Pulmonary embolism (PE) is probably the leading cause of death associated with liposuction. It occurs most frequently with general anesthesia or heavy intravenous (IV) sedation; unnecessary perioperative IV fluid infusion, causing hemodilution; and excessive liposuction.

Pulmonary thromboembolism is a major cause of death in surgery patients under general anesthesia for more than 30 minutes.1 A large prospective multicenter study found a 1% incidence of fatal PE among all general surgery patients.2 Another study of postoperative surgery patients found an 18% incidence of abnormal lung scans, consistent with PE.3

Reference: forceful extraction gesture

Reference information:
"Geste fort" seems to refer to the very aggressive physical gesture during liposuction with the cannula. I can't find any online reference for you but I think "forceful extraction gesture" acknowledges the reason for the pain after surgery, certainly a "con" to the procedure, without getting too graphic about why the traumatism occurs. It is indeed a very aggressive back and forth thrust with the cannula.
