Statement by employee/independent contractor recipient of State remuneration
I know this is a mouthful but I can't thik of any other way to translate the term as it is a totaly Italian thing ... basically it is a statement confirming that remuneration is being received from the State (i.e. Pubblica Amministazione) which does not exceed the remuneration levels established in the relevant legislation. Other, shorter translations, such as Declaration of Acknowedgement, Statement of Recognition, etc. would have to be explained in any case.
