В настоящее время в России лишь происходит формирование соответствующей законодательной базы, поскольку законы, которые действуют на сегодняшний день входят между собой в противоречие. Так, на публичное обсуждение вынесен новый проект федерального закона «О донорстве органов, частей органов человека и их трансплантации (пересадке)». Однако разработанный Минздравом законопроект вызвал жесткую критику среди трансплантологов: врачи прогнозируют, что в случае принятия законопроекта в нынешней редакции ситуация с и без того слаборазвитой системой донорства органов только ухудшится. В частности, медицинское сообщество критиковало сложную и многоуровневую процедуру регистрации волеизъявления человека – о его желании или нежелании становиться посмертным донором.

Something along these lines. In the US a person usually has to register as a donor if he/she wants to donate, that is to join an organ donor register. And this person has to sign a consent to donate form (consent forms may vary, as the person may change their mind later):

Actually, upon thinking, I suggest replacing "consent to donate" with "decision about organ donation", as more accurate in this context.
