This is from a survey given to doctors about new drug trials for a diabetes drug. I don't know what the question is and I don't know where the Spanish is from. Since it's just running list of responses, there's really no other context.
I found MC=motivo de consulta = reason for visit, but I'm not sure that it fits here.
