Full line reads "ctas a cobrar caja perif. pac" - I believe the "pac" refers to the patient, and the cts a cobrar to accounts receivable, but what is a "caja periferica" (If indeed that is what it stands for) in a hospital setting? For more context, the same item includes "Ctas a cob internados" and "Ctas a reclasificar"...

I'm guessing here but I'm hoping it helps anyway. I read the phrase you are having problems with in the same way as you. I think it's 'periférica' too.
I think this has to do with cash pooling where secondary accounts are set up. This may be a secondary account specifically for patients as hospitals will receive funds from various sources.
I've looked up 'peripheral account' but it only seems to be mentioned on the English pages of Italian and Spanish banks in the context of cash pooling so that's a bit suspect. I would therefore stick with 'secondary' as there's much more evidence of this being used in this context on English language pages.
