This is the title of a report/letter from a provincial government office in Spain. I've found plenty of examples in Spanish but can't seem to come across a translation. I think that Report and Conditions/Restrictions is along the right lines, but would appreciate any other ideas.

This is the title of a report-type letter outlining concerns about herbicide application in the region. It includes a list of conditions under which such application could be acceptable, but I am hesitant to use "terms and conditions" because it is not approving or authorizing herbicide use. Rather, I understand it is making recommendations to a higher body that will ultimately get to make the decision. Perhaps "assessment"?

Knowing the nature of the report and the reason for issuing it would help.

If the "informe" wasn't solicited by the body it's being sent to, "study" might be better (and more diplomatic).

information and terms/terms and conditions

There are a few explanations of the Web for "condicionado". As you thought it's just the conditions. I would use "information" instead of "report".
