French term

tulipe de désadaptation

French to English Other Medical (general)
Le tulipe de désadaption peut diminuer le risque d'accidents percutanés.
It appears that this device is used in the removal of a syringe or an injection pen. This is from a text describing a study on the risks posed by injection pens to health care personnel during the administration of insulin, etc. According to this study, most percutaneous accidents occur during "recapuchonnage-désadaptation de l'aiguille". Could désadaptation here mean removal?

needle disconnection/disengagement device

This requires a bit of explanation.

Old style syringe/needle systems consist of a syringe for sucking out or injecting in fluids and a needle that attaches to the syringe. The two items are detachable, permitting a variety of syringe sizes and needle lengths and thicknesses that can be mixed and matched according to the task at hand.

Typically, the needle is screwed into the syringe, both devices being threaded for this purpose. The needle itself is encased in a plastic cap that the user holds as he screws the needle into the syringe. If there were no cap, the needle would be contaminated by the user as he screwed it in and also, since the needle is so thin, he would not be able to grip it well enough to actually accomplish the screw-in as the connection is rather tight. Once the needle/cap combination is screwed into the syringe, the cap is removed by pulling it off along the axis parallel to the needle.

The act of removing the cap (uncapping as the asker has called it) poses no risk to the user, because the needle has not yet been inserted into the patient. Yes, it is still possible to stick yourself at this point, but this is of no consequence, because the needle has not yet been contaminated with the patient's (potentially infectious) blood.

The user proceeds to stick the needle into the patient, either to inject the fluid from the syringe or to draw out blood from the patient.

The needle is now removed from the patient and the dangerous part begins. If you try to recap the needle, you risk sticking yourself with the contaminated needle. If you try to disengage (desadaptation) the needle from the syringe without the cap on, you again risk sticking yourself.

In my office, the solution is to throw the whole syringe/needle combination into the sharps container without recapping it.

However, in the case of insulin pens, or Vacutainers(tm), the "syringe" section is not disposable, so you are obliged to DISENGAGE the needle before throwing it out into the sharps container.

The tulipe de desadaptation is a tulip shaped plastic device that replaces the cap and allows the needle to be attached and removed safely. The process is not uncapping the needle, but rather removing the needle from the syringe/pen/Vacutainer.

Thanks kashew. The flower of the tulip screws onto the pen and the stem is hollow and accommodates the needle.

Thanks Joanne. There are a variety of needle safety mechanisms out there, some complex and some very elegant.

A compact, substantially planar needlestick prevention safety device has a discoid shield-like portion, divided into an upper and lower half via a living hinge, and a pair of squeezable handles extending laterally therefrom. A central beveled aperture with outwardly extending radial slits accommodates within it a wide range of needle cap sizes. When the handles are squeezed by hand, the upper and lower halves of the disc fold toward each other along the living hinge and impart a variable and easily controllable amount of clamping force at the central aperture, thereby securely gripping the needle cap for uncapping or recapping purposes. The device provides a very high margin of safety during recapping, fits easily in the coat pocket for ease of accessibility, and is reusable and sterilizable by any sterilization method.

Note from asker:
thank you! "uncapping" would make a lot of sense in this context.
Peer comments on this reference comment:

neutral Michael Barnett : Liz, you are on the right track but "uncapping" is absolutely wrong.//What you describe is a pliars-like device for capping and uncapping needles. It does not work for insulin pens because the mechanism we are discussing does not have a removable cap

I know "uncapping is wrong" as I stated: this IS to do withdrawl of the needle, but is describing a special device that helps in withdrawing the needle...the asker jumped to the "uncapping" conclusion:)
