Primary diagnosis: bilateral embolic posterior cerebral artery infarcts

The patient's MR brain imaging revealed a large plaque within the left subclavian artery. The plaque had likely raptured with loca thrombosis and embolism of thrombus up his left vertebral artery leading to the infarcts.
We have decided against stenting the plaque and treated the patient empirically with anticoagulation.
His condition improved but he will need neurology follow-up. He clearly has residual impairment in verbal memory, and at this point it is difficult to say whether he will be able to return to work
