Both are supposed to sign a medical report. I used to use "doctor in charge" for both situations. Now that they come together in the same document, I don't know what to do!

This is how I look at it.
responsable = in-charge
encargado = substitute ( temporarily)

The first would be the physician in charge of the case in a general sense and the second would be the physican on duty at a given time--possibly.
