If it refers to the date of birth, this might be the most likely translation, meaning that is the Date of Birth transmitted to the Local Registar by either the parents or the hospital and registered as official.

By the way: \"Data in cui ricevuto/a dall\'Ufficiale di Stato Civile\", it is not wrong (apart from the missing double \"L\"), but it is not standard Italian. It is \"legalese/burocratese\" (law/burocracy jargon) for \"the date when something (which is here understated and of which it has been written before)has been/was received by.....\".

disagree  Luisa Calè: I am translating a Birth Certificate issued in the U.S. and I found the same phrase, it doesn't refer here to the date of birth declared by the parent, but as the colleagues said, it refers to the date when the document is recorded
