Im Laufe ihres Lebens kommt es bei den meisten CF-Patienten zu einer Kolonisation oder Infektion der oberen und unteren Atemwege mit Bakterien, Viren und Pilzen. Nicht alle Erreger werden nosokomial (d. h. im Krankenhaus) oder durch ambulante Kontakte von Patient zu Patient erworben. Bei einigen ist dies jedoch möglich oder sogar wahrscheinlich. Infektionen sind verantwortlich für akute, behandlungsbedürf-tige Exazerbationen mit Abfall der Lungenfunktion und Kom-plikationen. Infektionen mit bestimmten Erregern beeinflussen zudem langfristig den Verlauf, die Lebensqualität und die Langzeitprognose. Daher ist ein ausgewogenes Konzept der Infektionsvermeidung und Hygiene in Praxen, Kliniken, aber auch im häuslichen Umfeld sinnvoll und wichtig. Unabhängig von der Frage, ob bestimmte Infektionserreger primär noso-komial erworben wurden, spielen Erreger mit speziellen Resi-stenzen und Multiresistenzen gegen Antibiotika eine wichtige Rolle in der Diagnostik und Therapie-Entscheidung. Den ak-tuellsten und umfassendsten Überblick über den gegenwärti-gen Stand zur Hygiene und Infektionsprävention im deutsch-sprachigen Raum geben die Empfehlungen „Anforderungen an die Hygiene bei der medizinischen Versorgung von Patien-ten mit Cystischer Fibrose“, die von einer Arbeitsgruppe auf Anregung der Kommission für Krankenhaushygiene und In-fektionsprävention beim Robert-Koch-Institut, Berlin im Jahre 2012 erarbeitet und veröffentlicht wurden.1 Diese Empfehlun-gen sowie weitere wissenschaftliche Publikationen sind die Grundlage für die praktisch orientierten Empfehlungen dieser Broschüre.2,3,4
1.1. Übertragung
Die Übertragung eines Infektionserregers meint die direkte oder indirekte Weiterverbreitung des Erregers von Patient zu Patient. Während die direkte Übertragung – mit Ausnahme der aerogen über Aerosole übertragbaren Erreger – einen re-lativ engen Kontakt zwischen zwei Patienten voraussetzt, kann die indirekte Übertragung über die Hände des Behand-lungsteams und über kontaminierte Gegenstände (Medizin-produkte, Oberflächen) stattfinden.1
1.2. Standardhygienemaßnahmen
Standardhygienemaßnahmen sind Maßnahmen, die in medi-zinischen Einrichtungen im Kontakt zu allen Patienten ergrif-fen werden, um eine Übertragung von Infektionserregern auf den Patienten und die Mitarbeiter zu verhindern und das Ri-siko einer nosokomialen Ausbreitung von Erregern zu redu-zieren. Hierzu gehören die hygienische Händedesinfektion sowie spezielle Hygienemaßnahmen: 1
• Gebrauch von Schutzhandschuhen, Schutzkleidung
• Mund-Nasen-Schutz
• Desinfektion von Oberflächen und Gegenständen
• sachgerechte Aufbereitung von Medizinprodukten nach einem schriftlich festgelegten Hygieneplan
• räumliche (Segregation) und organisatorische Tren-nung von Patienten, die mit bestimmten Erregern kolo-nisiert oder infiziert sind
2. Bakterien
2.1. Staphylococcus aureus
S. aureus kann durch direkten und indirekten Kontakt über-tragen werden (kontaminierte Gegenstände, Oberflächen, Le-bensmittel, Tröpfchen). Wenn der Patient keinen Mund-Nasen-Schutz trägt, kann die direkte Umgebung von Patien-ten mit pulmonaler Exazerbation durch S. aureus kontaminiert werden.5 Ebenso durch patientennah eingesetzte Medizin-produkte, wie zum Beispiel Stethoskope und Inhalationszube-hör.6
Genetisch identische S. aureus-Isolate können über Monate bis Jahre in den Atemwegen persistieren. Langfristige oder dauerhafte Antibiotikatherapien (z.B. Cotrimoxazol) können wahrscheinlich die Bildung von so genannten Small-colony-Varianten, eine gegenüber Umwelteinflüssen und dem Ab-wehrsystem besonders widerstandsfähige Erscheinungsform, begünstigen.7
Vermutlich wird durch die Dauerprophylaxe mit einem oral verabreichten Cephalosporin die Kolonisation mit P. aerugi-nosa begünstigt.8 CF-Patienten, die mit S. aureus und P. ae-ruginosa kolonisiert sind und eine Langzeittherapie mit Azi-thromycin erhalten, können zu einem Reservoir für die noso-komiale Übertragung Makrolid- und Clindamycin-resistenter S. aureus sowie Makrolid-resistenter Haemophilus spp. wer-den.9,10 Der Anteil Methicillin-resistenter S. aureus (MRSA) an allen S. aureus-Isolaten hat bei CF-Patienten in den letzten Jahren zugenommen.3 MRSA kann zwischen Patienten und Haushaltsmitgliedern übertragen werden.3

During their lifetime, most CF patients suffer from a colonization or infection of the upper and lower respiratory tract by bacteria, viruses and fungi. Not all pathogens are acquired nosocomially (i.e. in hospital) or through ambulatory contacts between patients. In some cases, however, this is possible or even likely. Infections are responsible for acute exacerbations requiring treatment and are accompanied by a decrease in lung function and complications. Infections by certain pathogens also affect one’s long-term development, quality of life and long-term prognosis. Therefore, a balanced approach to infection prevention and hygiene in practices, clinics, but also in the domestic setting is reasonable and important. Independently of the question of whether certain infectious agents were primarily hospital-acquired, pathogens with special resistances and multi-resistances to antibiotics play an important role in the diagnosis and therapy. The guideline "Hygiene Requirements for the Medical Care of Patients with Cystic Fibrosis", published by a working group on the initiative of the Commission for Hospital Hygiene and Infection Prevention at the Robert Koch Institute in Berlin in 2012, provides the most recent and comprehensive overview of the current state of hygiene and infection prevention in the German-speaking area.1 These recommendations and other scientific publications form the basis for the practically-oriented recommendations included in this brochure.2,3,4
1.1. Transmission
The transmission of an infectious agent refers to the direct or indirect dissemination of the pathogen from patient to patient. While the direct transmission – with the exception of aerogenically transmissible pathogens via aerosols – implies a relatively close contact between patients, the indirect transmission can take place via the hands of the healthcare team and via contaminated objects (e.g. medical products, surfaces).1
1.2. Standard hygiene measures
Standard hygiene measures are measures taken in medical facilities with regard to contact with all patients in order to prevent the transmission of infectious agents to the patient and staff and to reduce the risk of a spread of nosocomial pathogens. These measures include hygienic hand disinfection and specific hygiene measures:1
• Use of protective gloves, protective clothing
• Mouth and nose protection
• Disinfection of surfaces and objects
• Proper preparation of medical devices according to a hygiene plan in written form
• Spatial and organizational separation of patients who are colonized or infected with certain pathogens
2. Bacteria
2.1. Staphylococcus aureus
S. aureus can be transmitted by direct and indirect contact (e.g. contaminated objects, surfaces, food, droplets). If the patient does not wear mouth and nose protection, the direct surroundings of patients with pulmonary exacerbation can be contaminated by S. aureus.5 Likewise, medical devices such as stethoscopes and inhalation equipment used in the vicinity of patients may carry S. aureus.6
Genetically identical S. aureus isolates may persist in the airways for many months or even years. Long-term or permanent antibiotic therapy (e.g. Cotrimoxazole) may encourage the formation of so-called small-colony variants, which are particularly resistant to environmental influences and the immune system.7
Presumably, the permanent prophylaxis with an oral cephalosporin encourages the colonization with P. aeruginosa.8
CF patients colonized with S. aureus and P. aeruginosa and receiving long-term therapy with azithromycin may serve as a reservoir for the nosocomial transmission of both macrolide and clindamycin-resistant S. aureus and macrolide-resistant Haemophilus spp.9,10
The proportion of methicillin-resistant S. aureus (MRSA) of all S. aureus isolates in CF-patients has been increasing over the years.3 MRSA can be transmitted between patients and household members.3
