Can someone shed some light on what "optique" means in this context? Is it just a way of saying a "souscription assurance vie" is what the client is planning on doing? This is in a document describing a financial institution's investor profile tool.

Yes for this sort of context I'd stick fairly close to the French: Marital status and family status must be completed when looking to subscribe to ...
Be careful with *assurance vie* which is basically a saving or investment plan rather than life insurance in the UK sense of the term. Life insurance in French is an *assurance décès*
