I'm asked to translate Contributor Release Form for appearance in video recordings and photos.

Shall I translate this word as コントリビューターリリースフォーム or 貢献者放映許可書, or any other way? Seeing that the release form is related to photos as well as videos, I wonder if it is safer to write them in Katakana because 放映 is for videos and films. In that case, is it normal to keep calling contributor as コントリビューター or change that to 貢献者 or is there any other appropriate word?
