Hi,
I am currently translating a hospital form which has the following title:
"Feuille Sommaire - Résumé de Dossier" = "Summary Sheet - Report Summary"
I don't particularly like the double "Summary"
Would anyone know if there is another name for this type of sheet? This is for a UK client.

If the form you are translating is from Quebec, then it is probably a discharge summary. That is how I have seen it translated, and that is how I have translated it over the years. I am providing two useful links, one from Quebec, the other from Europe. This doesn't solve your problem of two "summary"'s.

In genral, quite honestly, I don't think repetitions are a problem in hospital records in general.
