Depending on the type of surgery performed, pins or wires may extend through the toes, or a protective splint that extends over the end of the foot may be in place. Postoperatively, the foot is usually immobilized by a bulky dressing, short leg cast, slipper (plaster) cast, or a platform “shoe” that fits over the dressing and has a rigid sole (known as a bunion boot). The foot should be elevated with the heel off the bed to help reduce discomfort and prevent edema. Assess neurovascular status frequently during the immediate postoperative period. The devices inserted may interfere with or preclude assessment for movement. Also be aware that sensation may be difficult to evaluate because postoperative pain can interfere with the patient’s ability to differentiate pain from the surgery versus pain caused by nerve pressure or circulatory impairment.
