Seems to be a standard text as evidenced by this proz question,

neutral liz askew : why no references though? and I am unable to find this on any site./It would be nice were you to provide some then we can learn from this.? I didn't find any references./ Domiciliation note makes absolutely no sense to me in English.

I tentatively used 'domiciled bill' and I'm still waiting for confirmation or otherwise that this is indeed the case. The problem is compounded by the fact that the French talk of the 'bordereau' whereas I think the English would be more direct and say the bill itself. "
