Agree with Robert It is referring to treatment-naive patients. As the text is not clear, I would say: (A/The/This) treatment-naive patient would be taking more medication for his/her other condition with...(with what????)

"Naive" here has not been translated into Spanish, and might refer either to the patient's not knowing s/he is participating in a controlled study, or perhaps his/her lack of awareness of HIV processes and medications.

It is impossible to guess what came after "una formulacion" in these hastily scrawled or dictated physician notes. I would therefore suggest you leave "una formulacion" untranslated and let agency/client know the problem......

I think they mean "treatment-naive patient" here...I have found something to say this at the top of the spreadsheet!
