This is the title of a section of a CV for a doctor. My question is whether "assistenziale" implies volunteer/charity work, or simply health care work. The jobs listed seem to me to be health care jobs rather than volunteer jobs: frequentatrice ambulatori e reparto, dirigente medico, etc.

Every doctor needs to go through internship and later, if specialization is the goal, residency. Based on what follows in that section, you can decide whether he is talking about residency or internship experiences.

An alternative would be "assisting in special procedures" where he would list all his experiences assisting senior doctors in special procedures or the like. But you need to decide depending on the context.
