Immagino che si tratti di un documento redatto da un notaio, che quindi scrive riferendosi alla persona che - davanti a lui/lei - sta per sottoscrivere la richiesta di registrare la suddetta quarta integrazione. I riferimenti che trovo su Internet rimandano al Perù, dev'essere quindi un Ufficio non presente ovunque o con altri nomi. Credo che quella che ti ho fornito sia una traduzione sensata.

agree  Maria Assunta Puccini: "Extender" en este caso me parece que se refiera a la redacción de una escritura pública con base en el contrato o escritura privada que se le entrega al notario. ¡Que pases un super buen día!
