Под "Name" подразумевается, что фамилию и данные будут искать по компьютерной системе участковых врачей - General Practicioner (GP)

Кроме, как "история болезни" наверное есть какое-то более современное название. Участковый врач может быть лечащим врачом Диагностического Центра. Почему "covered" - в Росии это дело как бы само собой разумеется - к информации есть доступ далеко не для всех.

"Life inshurance" - будет 100% отказ, если человек болен, например, раком; "medical insurance" - поскольку предусматривает оплату лекарств и частных клиник(всё зависит с кем, с какой компанией - их много и у всех полис разнится) могут и отказать, могут поставить определённые рамки.

Соответственно и страхование программы - с больными людьми никто дела иметь не хочет, никто не будет подвергать программу риску срыва.

: История болезни = Case history. Health Plan - это программа медицинского страхового покрытия, определяющая кем, где и что именно покрывается.// Not really. A Health Plan is a commodity sold at a price which may depend on the existing Case History.
