The term is used in 'Besluit algemene regels milieu mijnbouw', where it is initially defined as 'ongewoon voorval: voorval als bedoeld in artikel 17.1 van de Wet milieubeheer.' Cross-referencing doesn't help much. Provisionally, I've translated it as 'reportable incident'. However, I would appreciate suggestions from anyone familiar with HSE terminology!

Aangezien een “Voorzienbaar ongewoon voorval” niet als een incident wordt aangemerkt, is het melden hiervan niet in het Bedrijfsnoodplan Shell Pernis opgenomen en is hiervoor deze aparte werkinstructie opgesteld in het BBS VGWM-ondersteuning en beveiliging.
