Does anyone know if there is a fairly standard English equivalent for this term in the field of health & safety legislation? Immediate context:
"Indien zich ... een ongewoon voorval voordoet of heeft voorgedaan waardoor nadelige gevolgen voor het milieu zijn ontstaan ..., meldt de uitvoerder het ongewoon voorval zo spoedig mogelijk telefonisch aan de inspecteur-generaal der mijnen"

The term is used in 'Besluit algemene regels milieu mijnbouw', where it is initially defined as 'ongewoon voorval: voorval als bedoeld in artikel 17.1 van de Wet milieubeheer.' Cross-referencing doesn't help much. Provisionally, I've translated it as 'reportable incident'. However, I would appreciate suggestions from anyone familiar with HSE terminology!

In addition, chapter 17 Wm specifies the measures that must be taken in the event that an unusual incident takes place or has already taken place. On the basis of this chapter, an unusual incident at an installation must be reported to the competent authority by the party concerned as soon as possible. At the same time, the party in charge of operating the installation must immediately take all measures that can be reasonably expected of him to contain the impact of the event, or, to the extent that such impact cannot be avoided, to limit and remedy it as far as possible.
