الأطفال لا يسقطون في الثغرات، إنهم يسقطون من بين أصابعنا
to the effect that people (caseworkers) are the ones responsible not the system, because there seems to be no such a thing as system (finger vs cracks).

I think that it would be better to render the meaning (the point the author wants to make is that only people are responsible for what may happen to a child, and that the system has nothing to do with this)
