OR:

failure to encourage expression of concerns/complaints on the part of participants

In such contexts, "concerns" comes across as a more polite/PC word.

This is the idea, I think. It seems to be a strange use of the word "instalacion".

These solutions should work in Brit as well as American.

Since it is being listed as a weakness, the expression in English has to be *negative* (and thus must include "failure to", "not', or similar).
