Da der Text mit "SCHEDULE", also "ANLAGE", beginnt und der fragliche Satz mit einem Doppelpunkt abschließt, denke ich, dass es sich hier um die Anlage zu einer Versicherungspolice handelt, deren Nummer gleich oben zu Anfang angegeben werden soll (hinter dem Doppelpunkt).
