The effects of gravity on the pulmonary system affect several aspects of pulmonary anatomy, along with other changes that involve ventilation, perfusion, and lung volumes. Positioning patients in the supine position increases the soft palate thickness and area. There is a decrease in the area of the oropharynx, the vertical length of the airway, and the anteroposterior dimension of the rib cage and abdomen. This is accompanied by increases in the lateral dimensions of these latter structures.
Supine Position. For patients in the supine position, the majority of respiratory function is the result of diaphragm movement. The diaphragm lies more cephalad when a patient is in the supine position, and in awake patients, the diaphragm moves caudad during spontaneous ventilation, with the greatest excursion posteriorly, so that the most dependent portions of the lung are the best ventilated. Because gravity also distributes pulmonary blood flow to the most dependent portions of the lung, ventilation and perfusion tend to be well matched. Under anesthesia, the diaphragm is displaced cephalad, but during mechanical ventilation the less dependent portions of the lungs tend to be better ventilated, worsening the ventilation-perfusion ratio. Nonetheless, because the height of the lung is lower when patients are in the supine position than when upright, ventilation still tends to be more uniform in supine patients than in upright patients.
Perfusion in the lung is defined by West zones 1, 2, and 3. Zone 1 is the least dependent portion of the lung (zone 1 is anterior when patients are supine), and because alveolar pressure exceeds arterial and venous pressure, there is no perfusion of this part of the lung. In West zone 2, pulmonary circulation is determined by the difference in arterial and alveolar pressures, and in West zone 3, where alveolar pressure is lowest, perfusion is a function of arterial minus venous pressure. A large portion of the lung is in West zone 3 when patients are in the supine position.6
Functional residual capacity (FRC) decreases while patients are in the supine position as a result of cephalad displacement of the abdominal contents against the diaphragm. Changes in vital capacity with assumption of the supine position are inconsistent, with increases, decreases, and no changes all being reported. Forced vital capacity and inspiratory capacity increase when patients are places in the supine position. Closing volumes, which represent the fraction of total lung capacity below which airway closure occurs, may increase slightly when patients are supine, thus contributing to a ventilation/perfusion mismatch, primarily in the more dependent portions of the lung.

Функциональная остаточная емкость легких (ФОЕЛ) уменьшается у пациентов в положении лежа на спине вследствие краниального смещения органов брюшной полости к диафрагме. Изменения жизненной емкости легких в этом положении не постоянны, отмечают как колебания в ту или иную сторону, так и отсутствие изменений. Жизненная емкость легких при форсированном выдохе и дыхательный объем в положении лежа на спине увеличиваются. Объем закрытия, являющийся долей общей емкости легких, ниже которой происходит закрытие дыхательных путей, может немного увеличиваться в этом положении, приводя к дисбалансу вентиляции и перфузии, преимущественно в нижних сегментах легких.
