I am trying to ascertain the difference in an itemized medical invoice between "honorarios medicos" and "derecho," which comes just below the medical fees, for roughly half the amount. Some extra duty charged to patients?

Just a guess/suggestion, based on knowing that fees charged in an invoice are 'honorarios' while the fees I have to pay to the immigration office are called 'derechos'. Perhaps the 'honorarios' represent the total fee and the 'derecho' represents the hospital or clinic fee.
