I would just translate it as "Please quote CERF 4056 in all correspondence". CE is probably "comisión ética", and RF could be ref.

Asker: You are totally right! The institution's name is Riesgo de Fractura SA so it definitely sounds like CERF stands for Comité de Ética Riesgo de Fractura. I don't know why I didn't think of it, but I'm very glad that you did! Thanks!
