This is my try:
Affidavit of parent: this day the father acknowledges the child as his own and he understands that his recognition of paternity/filiation herein established is not binding on the mother

or maybe I should use: affiant (parent)

Explanation:
A declaration is being made by somebody claiming to be the parent. As they could be lying, or just mistaken, English is not likely to call them a "parent" at this stage, just the person making the declaration. Unfortunately this is wordier than the French.

Explanation:
It is one of the formulas I see frequently in US birth certificates.
Otherwise, the word Declarer appears not specifying any parent.

Although I don't have a link to Google, I have here a document showing this exact words in the same context. The only difference is that the source language is Spanish and not French. The father's statement recognizing a child within a birth certificate.

"Declaring parent" is offered as an alternative, but personally I don't see your choice of "Affidavit of parent" to be incorrect. You could have said "Parent statement" but it may not be a standard word in such documents, at least not in the US.
